Job Description
Provides diagnostic, therapeutic, preventive and health maintenance consultative services to geriatric patients under the supervision of a geriatrician. Develops individualized interventions that improve the overall welfare of their patients. Responsible for completing comprehensive geriatric assessments to assist in diagnosing and monitoring chronic health issues and wellbeing. Engages in counseling, collaboration, and communication with families and patients in a sensitive manner to enhance the continuity of care for the elderly. Through a collegial and cooperative team approach, the Nurse Practitioner functions as an interdisciplinary team member in the delivery of high quality and cost effective care.
Basic Qualifications:
Active Wisconsin R.N. license or eligibility to practice professional nursing in the state of Wisconsin.National Certification (e.g. ANCC or AANP) as a Nurse Pr! actitioner.Current prescriptive authority in the state of Wisc! onsin (APNP certification).Current BLS certification.Board certification in Gerontological practice through ANCC or minimum of 5 years experience working with geriatric patients.
Other Qualifications:
Knowledge of commonly prescribed medications including dose and schedule, contraindications, precautions, side effects, method(s) of administration, food/drug interactions, drug/drug interactions.Physical, psychosocial/cultural, and spiritual assessment skills.Ability to make sound decisions based on assessment findings, convention and individual patient differences.
Exhibits patient and/or customer focus.Performs dependably and adheres to high standards of professional conduct.
License or Certification:
Exemption Status: Non-exempt
Hours/Pay Period: 80
Schedule Details: Monday-Friday; Day Shift. Schedule may vary based needs.
